From Penn Station take either the Montauk Line to Bridgehampton and then the South Ferry to Shelter Island and Rams Head via car or Ronkonkoma Line to Greenport, walk to the North Ferry and call Shelter Island taxi to arrive at Rams Head once on Shelter Island.
